How to add a header pic for your form?
After creating form and adding a few fields navigate to themes page. On the left side you can see Form Display settings and a field to upload files.
Add the image
Drag and Drop any image to the field and see it real time on the preview pane.
Click save
Click save and you are done, slick as a whistle
